Due to the statutory obligation set out in Article 21 of the Act of 13 May 2016 on counteracting the threat of sexual offences and the protection of minors (i.e. Journal of Laws 2023, item 1304, as amended) we inform you below of the obligation of students to obtain and submit information, certificates and declarations to the institutions where the student will carry out practical classes or professional practice with minor patients.
The information and certificates to be submitted should be no more than 1 month old from the date of commencement of practical classes or work placements. Students are advised to take action early enough to obtain the required information and certificates.
The student is required to submit the following information and certificates:
1) Information from the Register of Sexual Offenders (all the countries where you have live for the past 20 years),
2) Certificate from the National Criminal Register (PL: Krajowy Rejestr Karny - KRK) (all the countries where you have lived for the past 20 years), If the certificate is NOT in English then you need to provide its translation into Polish or English. The translation needs to be done by a sworn translator.
A certificate from the National Criminal Register (KRK) can be obtained in written form at the KRK Information Office located at ul. Czerniakowska 100 in Warsaw, or at one of the official information points. The list of information points is available here:
Please note: In order to obtain a certificate from the National Criminal Register (KRK), the application form must be completed with the relevant type of proceedings selected and the appropriate category of data indicated (like in the picture below).
3) Statement of the country or countries where the student has resided for the last 20 years
4) Declaration2 - original
5) Declaration3 – original (if it’s needed)
In case of residing in another country – information from the criminal record of the given country obtained for the purposes of professional or volunteer activities related to contact with children should be submitted. If the given country does not provide for the preparation of information from the criminal record, the student is obliged to submit the above mentioned declarations.

Please remember that a printout is not a document.
Polish documents: For certificates obtained in paper form, please submit the documents directly to the Dean's Office. In the case of a certificate from the National Criminal Register (KRK), the student should present the original for verification and copies.

In accordance with the official documents of WUM Authorities (listed below) regarding standards for the protection of minors at the University, students are required to submit the following information, certificates, and declarations to the Dean’s Office no later than September 30, 2025, prior to the commencement of their studies.
Failure to provide the Dean’s Office with the above-mentioned information, certificates and declarations within the above-mentioned deadlines will result in the student not being admitted to classes/internships due to the student’s fault.
If, for any reason, you are unable to provide the required information or certificates from foreign registers on time, you may submit a request to the Dean explaining the reason for the delay, how long it will take, and attach supporting evidence (e.g., correspondence with the embassy or other issuing authority).
